Patient-friendly isolation guidance sheet

Does anyone have a current, patient-facing one-pager for isolation and return-to-work (COVID/flu/RSV) that aligns with the CDC Isolation & Precautions app? Our ID clinic updated a simple PDF in August 2025 for the triage desk, but I’d like something clearer on symptom-based criteria and post-isolation masking to reduce mixed messages and prevent transmission during the first week back.

What’s worked for us is a bold first line — “24 hours fever-free (no meds) and symptoms improving; then mask for 5 days” — with a QR to CDC’s page so it stays current (https://www.cdc.gov/respiratory-viruses/guidance.html); we add a small footer to extend masking to day 10 for high‑risk units — want the editable PDF?
